Not Every Outstanding Balance Needs the Same Fix.
SWIFT BILL starts with the claim history and payment records to identify why a balance remains open and which action the evidence supports.
A Claim That Never Reached Payment
Our team reviews rejection messages, denial reasons, and available records to identify billing errors, missing information, or payer decisions that may warrant further review.
An Unpaid Claim With No Clear Status
Payer status checks and submission history help us determine whether a claim was received, is pending information, or needs follow-up. The findings guide the next action.
A Payment That Does Not Match
We compare remittance details, posted payments, and applicable contracted rates when available. Potential underpayments and posting discrepancies are flagged for investigation.
A Balance Left Behind
Previous billing notes, payer correspondence, and unresolved requests help us reconstruct what has already been tried and identify work that still needs attention.
ABA Details Matter.
The review may include authorization references, rendering-provider information, service dates, billed units, and modifiers. Any proposed correction must be supported by the underlying records.
Start With the Claims That Still Have a Path Forward.
Aged A/R recovery begins with a focused review. The aim is to prioritize supported action, account for deadlines, and make the work manageable.
-
Review the Backlog
SWIFT BILL reviews A/R aging, claim history, remittance information, and payer correspondence to separate payment issues from recordkeeping issues.
-
Set Recovery Priorities
Our team considers filing and appeal deadlines, documentation, payer status, and potential recovery. The claims, work, and fee are agreed before recovery follow-up begins.
-
Work the Agreed Claims
Depending on the issue, we submit supported corrected claims, request reconsideration, prepare appropriate appeals, or follow up with the payer.
-
Reconcile the Outcome
Payer responses and payments received by your practice are matched to the claims worked. We explain remaining balances and unresolved barriers.
See the Work Behind the Balance.
An A/R total alone does not explain what has been done. SWIFT BILL documents claim activity and separates payments received from adjustments, write-offs, and amounts still outstanding.
Organized Records. Fewer Repeated Steps.
Digital claim tracking and structured worklists keep payer responses, follow-up dates, and supporting records connected. That gives our team a clearer basis for action and your practice more useful updates.
What Your Updates Cover
- Claim status
- Where each assigned claim stands, based on the records and payer responses available.
- Actions taken
- Contact dates, reference numbers, corrections, and review requests.
- Financial outcomes
- Confirmed payments, balances remaining, and supported adjustment recommendations.
- Open issues
- Missing records, pending payer decisions, and the follow-up still required.
Reporting format and frequency are agreed as part of your service scope.
A Defined Project. A Written Quote.
Pre-existing A/R varies in age, volume, documentation, and prior follow-up. SWIFT BILL quotes recovery work separately from ongoing ABA billing, based on the agreed backlog.
Payments Go to Your Practice.
Insurance payments are made to your practice. SWIFT BILL tracks the claim work and records confirmed payment outcomes within the agreed scope.
Your Proposal Defines:
- The claims, payers, and dates of service included
- The review, correction, follow-up, and appeal work covered
- The fee structure and how charges are calculated
- The reporting schedule and engagement period
Work outside the agreed scope is discussed and priced before it is added.
Understand What Recovery Can Involve.
Claim age, payer rules, and available records all affect the options. These answers explain how we approach the work.
How old can a claim be and still be reviewed?
Age alone does not determine recoverability. SWIFT BILL considers filing and appeal limits, submission history, proof of timely filing, and any applicable exceptions. An older balance may have options, but some deadlines may already have closed.
Can every unpaid claim be recovered?
No. Payment depends on factors such as coverage, authorization, documentation, payer rules, and deadlines. We explain supported recovery opportunities and identify balances that may not be collectible. A reduction in A/R from an adjustment or write-off is reported separately from money received.
Can you review claims handled by another billing company?
Yes, subject to access to the relevant records. Our review considers prior submissions, payer correspondence, payment posting, and follow-up notes. We agree on responsibilities with your practice to help avoid duplicate submissions or overlapping work.
Will every denial need an appeal?
No. Inaccurate claim information may call for a corrected claim. Disagreement with a payer decision may require reconsideration or an appeal under that payer's process. SWIFT BILL selects the supported response based on the issue and available records.
How long does old-claim recovery take?
There is no single timeline for an entire backlog. Record availability, claim volume, the action required, and payer response times affect progress. We set a reporting schedule for the engagement and explain pending items and delays.
Does this service include collecting patient debt?
This service focuses on unresolved insurance claims. Patient debt collection is not included in the old-claim recovery scope described on this page.
Take the First Step Toward Resolving Unpaid Claims.
Describe the backlog and where follow-up has stalled. SWIFT BILL will explain whether the work fits our recovery service and how we would scope the engagement.
Please do not include patient information in the contact form.
